Medical Director
An essential piece of our hospice care model is our Hospice Medical Director, who works hand in hand with the patient’s attending physician. 24-Hour Telephone and In-Person Support

That is why we provide 24-hour telephone access to hospice-trained nurses who can offer guidance, support, and, if necessary, arrange an in-person visit. Whether you are worried about a change in a patient’s symptoms or need emotional reassurance, our team is just a phone call away.
In-person visits are tailored to the patient’s specific needs, which may change as the disease progresses. We strive to maintain flexibility in our scheduling while preserving the patient’s comfort, dignity, and peace of mind.

Under the guidance of our Volunteer Coordinator, these caring individuals step in to offer companionship, help with errands, facilitate crafts or light activities, and provide family caregivers a much-needed break. Their contributions are heartfelt and invaluable, bringing extra joy and comfort to patients’ lives.Our volunteers come from diverse backgrounds but share a common commitment to compassionate service. They understand that a quiet conversation, a shared story, or a simple card game can be incredibly meaningful in a patient’s final days. Their role in supporting our in-home hospice care cannot be overstated, and we continuously celebrate their generosity of spirit.
Spiritual Care/Bereavement Spiritual and emotional well-being are core elements of end-of-life care.
Our spiritual care counselors aim to nurture patients’ beliefs, values, and traditions. We understand that spirituality is deeply personal, so we respect each individual’s journey without imposing any particular religious perspective. If requested, our team can coordinate with a patient’s own clergy or faith leader to ensure that the patient’s spiritual needs are being met.We also offer bereavement support for families, recognizing that grief and loss do not end at the moment of a patient’s passing. For 13 months after a loved one’s death, we continue to provide check-ins, resources, and a compassionate shoulder to lean on. Whether through phone calls, home visits, or memorial services, we remain present in the lives of the families who have entrusted us with their loved one’s care.
By guiding families through these important decisions, our social workers help remove stress and allow patients and their loved ones to focus on what truly matters: sharing meaningful time together.

Hospice Aides and Other Professionals
Our hospice aides offer hands-on support with personal care tasks such as bathing, dressing, grooming, and meal assistance. They function as an extension of our nursing services, ensuring patients maintain the highest level of comfort and hygiene.
We also have access to physical, speech, and occupational therapists for specialized interventions, as well as dietitians for nutritional assessments.Our homemaker services can assist with housekeeping tasks like changing bed linens or emptying trash, helping maintain a clean, safe environment.
