What Our Hospice Care Services Include
Grace and Glory Hospice provides a comprehensive range of hospice care services focused on comfort, dignity, and quality of life. Services are personalized and adjusted as needs change, ensuring care remains responsive and supportive over time.
Our hospice care services are available 24 hours a day, 7 days a week, and are 100% covered by Medicare and Medi-Cal for eligible patients.
- Medical Director. Our Medical Director clinical oversight and works closely with the care team to guide comfort-focused care. This role helps ensure care plans align with best practices while remaining centered on the patient’s goals and preferences.
- Registered Nurses. Registered Nurses are central to hospice care. They assess comfort needs, coordinate care, provide education, and serve as a key point of contact for patients and families. Nursing support is available around the clock, ensuring guidance is never far away.
- CNAs and Hospice Aides. Certified Nursing Assistants and Hospice Aides provide hands-on support with personal care. Their presence helps patients remain comfortable while supporting daily routines with dignity and respect.
- Social & Spiritual Care. Social Care services focus on emotional support, practical guidance, and resource coordination. Spiritual Care is available for those who desire it and is always respectful of individual beliefs and values.
- Wound Care. Wound Care support is provided as part of comfort-focused nursing care when appropriate. This service is delivered thoughtfully, with attention to comfort and skin integrity.
- Volunteer Support. Volunteer Support adds an extra layer of presence and companionship. Volunteers may offer emotional support, a calming presence, or practical help, based on availability and individual preferences.
