Hospice Care Services Available in Alameda County
Grace and Glory Hospice provides comprehensive hospice services throughout Alameda County. Our care is designed to manage symptoms, reduce suffering, and support both patients and families during this important time.
All services are coordinated by our clinical team and delivered wherever you call home; whether that’s Oakland, Fremont, Berkeley, or any other community in the county.
- Medical Director
A hospice physician oversees your medical care and works with your existing doctor to ensure symptom management and comfort. Medical direction is available 24/7. - Skilled Nursing
Our registered nurses make regular visits to assess symptoms, manage medications, provide wound care, and coordinate your overall plan of care. You’ll have a primary nurse who knows you and your family. - Pain and Symptom Management
We provide wound care and expert care to manage pain, shortness of breath, nausea, anxiety, restlessness, and other symptoms related to your hospice diagnosis. - Medications and Medical Supplies
All medications related to your hospice diagnosis are provided and delivered to your home. We also supply medical equipment such as hospital beds, oxygen, walkers, wheelchairs, and other items needed for comfort and safety. - Personal Care Assistance
Certified hospice aides visit regularly to help with bathing, dressing, grooming, and other activities of daily living. This support gives family caregivers time to rest. - Social Work Services
Our medical social workers help with advance care planning, community resources, insurance questions, and emotional support for the entire family. - Chaplain and Spiritual Care
Spiritual support is available to all patients and families, regardless of religious background or belief. Our chaplains provide comfort, guidance, and a listening presence. - Family Support
We provide education and training so you feel confident providing hands-on care at home. If you need a break from caregiving, we can arrange short-term respite care (up to five days) so you can rest. We also offer counseling and emotional support for the entire family during hospice care and bereavement services for up to 13 months after your loved one’s passing.
