Compassionate Hospice Care, Right Where You Are in Berkeley
Grace and Glory Hospice is a CHAP-Certified, family-owned hospice proudly serving Berkeley and the surrounding Alameda County community. Our Director of Nursing, Jane Porter, founded this hospice after 20 years as a hospice nurse because she believed families deserve care that is personal, attentive, and rooted in genuine human connection. That belief guides every decision we make and every home we walk into.
When you call us, you reach people who know your loved one’s name. You won’t navigate a call center or repeat your story to a different person every time. You get a consistent, compassionate team that shows up and stays with you.
What Hospice Care in Berkeley Includes
When your loved one begins hospice care with us, a dedicated interdisciplinary team comes directly to them, wherever home may be. Under the Medicare Hospice Benefit, covered services typically include:
- Registered Nurse visits for ongoing symptom management and clinical oversight
- Physician and Medical Director coordination with your loved one’s existing care team
- Certified Nursing Aides (CNAs) for personal care, bathing, and hygiene support
- Social work services for care planning, emotional support, and community resources
- Chaplain and spiritual care that honors your family’s beliefs and traditions
- Bereavement support for family members, both before and after the loss
- Medications, medical equipment, and supplies related to the hospice diagnosis
- Respite care to give primary family caregivers time to rest and recharge
Every plan of care is tailored to your loved one’s specific needs, values, and goals. Care is never one-size-fits-all.
Serving Berkeley’s Diverse Community
Berkeley is one of the most diverse and educated cities in the Bay Area, home to residents from many different cultural backgrounds, nationalities, and walks of life. It is a city where independent thinkers, long-established families, university communities, and working-class neighborhoods exist side by side. That diversity shapes how people approach health, family, decision-making, and end-of-life care.
We approach every family with genuine respect for their background, values, and the way they define what matters most. Whether your family holds deeply spiritual traditions, secular values, or something entirely your own, we meet you where you are without judgment.
Whether your loved one lives in North Berkeley, the Elmwood, West Berkeley, Thousand Oaks, or the Claremont hills, our team comes to them.
Who Qualifies for Hospice Care in Berkeley?
According to Medicare guidelines, a patient is eligible for the Medicare Hospice Benefit when:
- A physician certifies a prognosis of six months or less if the illness follows its expected course
- The patient, or their legal representative, chooses comfort-focused care over curative treatment
- The patient is enrolled in Medicare Part A
Hospice is appropriate for many serious illnesses, including heart failure, COPD, cancer, dementia, kidney disease, liver disease, ALS, and Parkinson’s disease. You do not have to be in the final days of life to qualify. Many families tell us they wish they had started sooner.
If you are unsure whether your loved one qualifies, we offer no-obligation care evaluations. We will come to you, learn about their current situation, and give you a clear, honest answer.
